i think knowledge based medical profession are losing, financially speaking. only those skill based like surgery, or anaesthesiology are generating high income

the traditional education system use "honours" (bachelor, master, phd) while the neo education system uses badges (coursera, edx) . uniquely few private universities in Malaysia are combining the best of both world, like the pics i shown below: image

in Malaysia, the doctor hierarchial system is quite gapped. Like this 1. House officer 2. Medical Officer (junior medical officer) (senior medical officer) (resident / gazzete) 3. Specialist (senior specialist) (consultant) only the one with numbers really shows a good salary jump and legally different than the previous stage. while the one in brackets are just unofficial classification.

for those wondering what happened, well i resign from an academic post at a government university for multiple reason, in large part because my phd student loan was rejected and in part because my autoimmune disease had become worse. unfortunately, my university post has a clause which basically say "if i resign without a phd or before a bond of 8 years, i gotta pay all my salary back". now, when we resign, our last month is usually blocked from being given to us (i dont know, its a malaysia thing), but because of the bond, they wont be paying that salary to me. hence.... that what happens. image : the online system showing my salary got blocked. sepenuh masa : full time employee bulan : month 2024-02 : February 2024 jana : generate pemberitahuan : notice tiada maklumat gaji : salary info not available image

With the government now only employing new doctors on a contract basis (absorption to permanent is very slow), a lot of doctors opted to work in the private sector. unfortunately, the Malaysian healthcare system does not allow specialist training in the private sector for nearly all field. As such, most doctor can only work in two fields : family medicine (general practitioner) or the emergency department. Consequently, a lot of general practice clinics are opened everywhere. Competition become so tough, especially in cities like Selangor or Kuala Lumpur. More clinics are closing every year due to low income. Wages are going negative. To stay relevant most clinics attempted to provide service plus. Unfortunately, at times this come at the price of ethics.
