The biggest mistake people are making with the #bookstr publishing is formatting. The main formatting should just be the "= " book header and "== " chapter headers.
I think you might be using document converters, or something, but you're best off just copy-pasting the content as-is and putting the equals signs in front of the lines you want to be a book or chapter header.
After it is printed as events, you will see how it looks and add can formatting and reprint (that overwrites). It's an iterative process of editing.
