The Knots 28.1 tag differs from Bitcoin Core 28.1 by 1,370 commits and 28,511 LOC (+25,871, -2640) across 550 files.
I was told that Knots is basically Core, but with filters. Are the above numbers an amount that would correlate to the just the filters or are there other changes as well?

More configurability I would say.
There are a number of things that Core have done to make it a little hard to maintain forks of it, like hard coded names distributed throughout many files. It requires additional changed files and sometimes more iterative testing and revision after multiple commits. I don't know the full scope of the differences though. You can inspect them yourself with a git diff between the two forks.
