It requires active approval. In the event I have linked above, you can see there's a tag that with "bad" as value. Only nodes with 38425 events with "lgtm" tag value are accepted by routstr clients. Every other node (including nodes without any events) are disabled by default. It's my responsibility to ensure new nodes slowly get adoption (still unclear about the official process but yeah). It is sorta censorship, but users have the ultimate control ofc.
