Afaik one guardian of the federation lost access to their domain. The the domain was somehow contained in the data that got hashed to the federation ID, so they couldn't just swap out the domain. So the federation was still working with the remaining guardians, but really slow and unreliable. Fedimint fixed this now, so it shouldn't happen again.
