Perhaps it is a scan out of those terrible gray-ish paper booklets people get in the mail or at the post office, and not a digital form

Still skeptical. Every IRS form and worksheet is numbered and published online. The OP says it accompanies the the1099-DA. No mention of this supposed worksheet in the 10-page instructions.
