Codex has been annoying in this regard as well. It kept treating otp codes as secrets and refuses under any condition to display them even when they expire after a short duration and the accounts are fully in my control. The shift to open source models cannot come fast enough

I think that's a different problem. But not sure if it's worse or better, probably worse if that's deeply hardwired.

Yeah it probably is. I also noticed that when this was triggered while using the codex cli, it kept displaying a big notice about responses becoming slower as a result of cyber risk observed in the session with a link to God knows what. It just fell off and weird .
