I won't give the answer that 100% confirms it since then thats to easy to fix next time. So let me give the specific answer for this image. The photo may be real, the shoe definately isn't. Look carefully where it is placed on her head. You see that its a bit forward, if you did that IRL it would fall off.
Actually I can say it since "she" already found out. There was a very visible google gemini watermark in the bottom right corner. The post has now been edited with it blurred over.

It can be either. I find the picture very coherent except for the shoe. So I expect that it was a real picture but not the same person as the account. So they had to get creative and asked AI to add the shoe.
